Childhood Cancer Awareness special license plate

The Childhood Cancer Awareness Special License Plate created by House Bill 18-1255 is effective Jan. 1, 2019. A one-time donation to the Morgan Adams Foundation (https://www.morganadamsfoundation.org/licenseplate/) is required to qualify for the Childhood Cancer Awareness Special License Plate.
 The Childhood Cancer Awareness special license plate has opted in for the DRIVES Certificate PIN (a unique identifier) and can be presented on any form that the nonprofit has provided to the customer.
The Morgan Adams Foundation will issue a certificate to qualified individuals. This certificate is only valid with a DRIVES Certificate PIN and may not be used by itself without a DRIVES Certificate PIN. This certificate can be presented in either a physical copy or electronically.
Personalized:
• Yes via https://mydmv.colorado.gov/?Link=PersonalizedPlate or in person at a county motor vehicle office. A one-time fee of $60 will be charged upon approval and a $25 fee will be collected each year thereafter. The logo will be removed from all personalized plates.
Plates can be issued to:
• Passenger cars, trucks, motorcycles, and noncommercial or recreational motor vehicles that do not exceed 16,000 pounds. This plate will be issued through the print on demand process.
